We just received form I-797C, Notice of Action, for my wife.

She is scheduled for an interview in about a month, and we're looking for a civil surgeon in the area who can help us with form I-693.

We have all the vaccination records in DS-2054, DS-3030, and DS-3025 - looks like we just need to ask the civil surgeon to fill out form I-693.

When I called into Palo Alto Medical Foundation to make an appointment with civil doctor in Sunnyvale, CA, we were quoted for $300-700 out of pocket because insurance would not cover it.

Wow... that's pretty expensive. Is this a normal cost to get I-693 filled out?

I used this Civil Surgeon,

I payed $175 but this was including tb test and varicella vaccination, they filled out the I-693 and put it a sealed envelope.

Not too bad of a price. :thumbs:

I would say this is extremely high! Most places it is between $10-$50 to get the records transcribed.

There is an old post that had a list of civil surgeons (you will have to rifle through it though)

Hi

I too received the I - 797C and one of the requirement is submission of I-693, my question is do we need to submit the I-693 even if we have already done and submitted the medical tests/reports in the beneficiary's country (Australia) before entering US. Also while filing I-485, I submitted DS-3025 to USCIS.

If we do have to submit I-693 does that mean I have to redo all the tests etc., or does the Civil Surgeon complete the form based on the medical results of the previous tests.

Thanks for your advice, everyone. So the $300-700 price tag was actually high even by the bay area standards. Everyone seems to be looking for any opportunity to gut their clients here.

I skimmed through the aforementioned thread, and will look into contacting Dr. Gomez in San Jose, CA.

Even though we submitted vaccination records like DS-3025, we did not actually submit I-693 yet. Since DS-3025 lists all the vaccinations that my wife has already received, I am hoping that our civil surgeon will just administer whatever vaccination that she still needs.

I had three immigrants who all needed vaccinations that weren't given at the medical in Vietnam. I also decided to go ahead and have the civil surgeon do the full medical and fill out the entire I-693 for everyone. Occasionally, the medical packets get lost between the POE and USCIS, and I'm allergic to RFE's and didn't want to take any chances. I paid $450 for all three full medicals, including vaccines. The civil surgeon was Joseph Ippolito. He's got offices in Oakland and Hayward.

$300 to $700 for one person is very expensive.

The I-693 is not required for K-1 who got their medical exam less than a year ago, and whom vaccination record is showing as meeting all the requirements. You submitted a copy of your DS-3025, which is good, and you did not receive a RFE? So you're good to go, nothing to do on that part.

The interview letter for AOS is generic for anybody adjusting status, and is not specifically worded for K-1.

The interview letter is a general form letter sent to everyone, not just those adjusting from a K-1. If the DS-3025 was filled out properly and you had all the required vaccinations at the time of the K-1 medical, then no need to rush out and get an I-693 for the interview. I hope all enjoyed their Thanksgiving Day weekend.

I just called into Meridian Medical Group this morning, and my wife still needs the whole MMR vaccination, not just Rubella (for which she already received shots), and extra flu shot since we're in the flu season. They were the most generous people so far, estimated $145 for the entire physical including immunization. We'll see if anything changes when we go in tomorrow.
