I also had mine done. U Wud need to go the high court for an affidavit of name change but before that u hv to publish it on a newspaper first, u wud be asked that in court. Also hv ur marriage certificate &passport pix handy.I'm also thinking u need someone over 40yrs of age to witness for u in court(not sure again)but I remember being asked but later someone helped me out. Hope this helps

http://www.courts.ca.gov/1051.htm put name change in the find box.

Each state has its rules about how to change name. Type in your state with name change court forms and see what you get. I am enclosing California requirements for you to compare. I hope this is somewhat helpful. Good luck!

I went to my ss office and showed them my marriage license along with my birth certificate and id, they sent my ss in the mail in one week then I took my ss card with my new last name to my dmv for my new driver license and that is it. your name is change thru your ss office and your dmv, no need to go to any court. Im in nc so that is the rules for this state.

"If you recently got married and want to change your last name to your spouse's last name, you may not have to go to court. Go to your local DMV office and your social security office, and ask them whether you can change the name on your driver's license and social security card to your spouse's last name without a court order. Sometimes, they will change your name if you show them your marriage license or certificate and you will not have to go to court. But it is possible they will tell you to get a court order, so be prepared for that." http://www.courts.ca.gov/1051.htm

Ultimately, the procedure depends on the issuing office of whichever document you are trying to change. It shouldn't be hard to find out on the phone or online. For example, to get a US passport in a different name (I used to work in a office that processed passport applications), one would need to submit a certified copy of the marriage certificate OR the court order OR the divorce decree that shows them going from one name to the next. So you shouldn't need a court order if you are using your marriage certificate!

I imagine it's probably the same for a driver's license to just submit the marriage certificate, in your case, along with the application. But check your dmv online to make sure.

I am also working along the name change line and from my little research, I think NY is the same. You go to SS office with marriage cert (now i read birth cert so hope i dnt forget that later) , then after iu get the new SS card, go to DMV and then of course banks, schools, state boards,Human resources at my job, passport office, health /life insurance, etc...phew!!!
