1 )We moved at end of August. I need to do an AR-11 and then do AOS paperwork. It asks if we have a case pending. If so, it asks for Receipt #. How do I know if we have a case pending? I am US, we did K1 fiance visa. Approved in Nov 2012, married in Las Vegas Feb 2012. Just now getting around to filling out paperwork, but need to do the address change first and can't figure out how to do that.

2)AOS process... G325a... Is this on me (US Citizen), her (my Colombian bride), or both of us?

3)Since we married in Feb 2012, do we still use her maiden name or do we put my last name now? Other than the marriage certificate she has no US docs. Ie. We were a day late trying to get a SS# (so no SS#) and you can't get a driver's license or ID card in my state without a SS#, so we have nothing with her name on it (other than our new lease from Sept 1, 2012... which we put her last name as mine).

4)DS-3025 Vaccination Document Worksheet was done in Bogota on 11/8/12, so we are outside 1 year. Numerous vaccines are filled in on this and he gave her some dated 11/8/11. Futher, the box he checked at the bottom says "Vaccination incomplete" and also "Applicant maybe eligible for blanket waiver because vaccination not medically appropriate." Perhaps she will have to do another medical in the US since we are at 1 year and 1 day? Regardless, should I go ahead and send this in and just let them RFE us if a new medical is needed?

Thanks.... Someone please answer question #1. Ie. Do we have a case pending or not? That question is confusing me, thus the reason I have not yet started on the AOS. I have had time on a few occasions in the last 3 months to do the AOS paperwork and I always stop at filling out the AR-11 because I don't know what to do since I don't know if we have a case pending or not.

When you file your petition just use your new address. Here is a link to the guides on VJ http://www.visajourney.com/content/k1k3aos

The only way you would have a case pending is if you have filed any petition since you spouse arrived here on the K1 visa.

Please review this guide, it will help you with some doubts you may have.

According to the USCIS website, you can also change your address online.

http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=c1a94154d7b3d010VgnVCM10000048f3d6a1RCRD&vgnextchannel=db029c7755cb9010VgnVCM10000045f3d6a1RCRD

1) You said that "Approved in Nov 2012, married in Las Vegas Feb 2012." How that happened? :bonk:

After you do the change of address, maybe you can call USCIS and ask them if they updated the new address in the system or if you are ready to go to send the papers for AOS (from k1 visa). I never had to change my address when I entered US.

3) When you start filing all the forms for the AOS (please read the guide link), you have to include the last name she will use from now on in USA. If she want to have the married name in all her US IDs (SS#, GC, EAD and AP) then she has to include it in the forms, otherwise she can still keep using the maiden name. It's up to you guys.

1. You do not have a case pending.

2. You can use her married name on the forms, if that is the name she wants to have. You don't need other docs to prove name change, the marriage cert will do that for you.

1) We're going to use my last name on the AOS forms. Should we use her last name on the AR11 (change of address) that I am currently filling out online? I assume since they have her last name in the system currently I should put that last name on the change of address form and then use my last name on the AOS forms (showing the change in name). Good idea?

2)On the change of address form it asks for her "A" number. Does she have an "A" number? Is that the admittance # on her visa that is stapled inside her passport? Oops... there is also a 9 digit number following an "A" on the back of that Visa that is stapled inside her passport. No clue which of the 2 numbers to use.

1. You do not have a case pending.

2. You can use her married name on the forms, if that is the name she wants to have. You don't need other docs to prove name change, the marriage cert will do that for you.
