A civil surgeon in your area can complete the I-693 for you. It needs to be signed, sealed by the surgeon and sent in with the rest of the AOS forms.

Use Google to find a civil surgeon near you.

I should also note that they will need the vaccination worksheet provided to you during your medical in order to prepare the I-693!

To find an authorized civil surgeon, you have to use the USCIS website. In case you decide to use a civil surgeon, before you go make sure that doctor is still working with USCIS.
