We are just about ready to put our completed AOS forms in, we are nervous we have missed something out, we have read the insructions over and over but would be grateful if someone could double check. Also I am confused because I understand K1 people who have had a medical in their own country do not need to submit the medical record (I think it is I693) but I have just seen another K1 person on this site which listed this medical form as one that they are submitting. I thought I could just send in a copy of my vaccination record.

These are what we are planning to submit :-

1)G32 Bio information

2) I485 Application to register permanent status

3) Copy of my birth certificate

4)copy of certified marriage certificate

5)2 colour photos of me as per specifications

6)copy my passport with visa and my details pages

7)I864 Affidavit with all back up evidence

8) I797 approval notice for my K1 (copy)

9)vaccination worksheet and certificate of immunization (which medical people in London gave me)

10) I131 Travel froem Optional

11) I765 employment optional

This was our cover letter, If you want to include the DS-3025 only thats totally up to you.Some people don't want to chance an RFE by not having the I-693. As for the I-131,you will include the same documents as the I-765. Good Luck :)

You need a copy of your I-94 front and back

Photocopy of Passport (biographical page as well as entry stamps)I suggest the entire passport- there are water marks on each page and a copy of the entire passport is best :)

I sent my AOS/EAD/AP package off a couple of weeks ago. I only included one copy of my passport biographical page, K1 visa and I-94. Should I have included three copies of each? Will that be a problem?

A civil surgeon in your area can complete the I-693 for you. It needs to be signed, sealed by the surgeon and sent in with the rest of the AOS forms.

Use Google to find a civil surgeon near you.

I should also note that they will need the vaccination worksheet provided to you during your medical in order to prepare the I-693!

You don't need another medical completed. The civil surgeon will be able to complete the I-693 without it. If they refuse to do it for you unless you get another full medical completed, find another surgeon! :) My civil surgeon tried to convince me to get another one done in case the one sent to USCIS was lost but why worry about that if it isn't lost? You can just go get another one if worse comes to worse.

You don't "need" the I-693 if your shots were complete. If they aren't you at least need a vaccination transcription but the way the form is worded now doctors tend to try and scam people into entire new medicals.

As for the I-131,you will include the same documents as the I-765. Good Luck :)

Since each of the I-485, I-765 and I-131 are separate applications I made sure each had the appropriate documents and as you see I had no RFE and received my Green Card in less than 5 months of filing AOS. Better safe than sorry I say :)

Yes you're right. Better safe than sorry which is why I would provide the CORRECT documents. You stated "As for the I-131,you will include the same documents as the I-765." which is incorrect. The I-131 also requires a copy of the K1 visa which is not a requirement of the I-765, so they would be missing a document. Some people get away with missing documents, others don't. Better to include what's required.

As always, read the instructions, but the AOS guide in my signature is complete with notes, examples and links to more info. It won't steer you wrong (except the filing fee of $1070 is mentioned as $1010 and it'll probably change again soon).

Although sent together with the I-485 the I-131 and I-765 should be each treated as a separate packet with the required documentation for each one.
