Law #285-04 (Ley General de Migración) & Law #136-03 (Ley que crea el Código del Menor) Establish all requirements needed by a minor in order to leave national territory.

If the child is Dominican you'll need:

Original Dominican birth certificate legalized & a copy (RD$200 in extensa & RD$200 for legalization)

copy of passport bio page both for the child & the accompanying traveling adult

copy of parent's cedula if Dominican or passport bio page if foreign of the authorizing parent

copy of visa if applicable

Two 2x2 photograph of the child frontal view

Signed & notarized Affidavit drawn up by a court recognized Dominican lawyer (RD$1000)

The signed & notarized Affidavit (letter of authorization) needs to be taken first to La Procuraduría General de la República (PGR) to get the lawyer's signature verified (RD$340). The now verified Affidavit by PGR if taken to La Dirección General de Migración with all other supporting documents The manor exit permit will be issued the same day in a few hours (VIP service) for RD$2000. Permits were issued in 3 working days for the cost of RD$1000 but that service is no longer available, all permits are now issued VIP (in a few hours for double the cost of RD$2000).

Also here is the link from DR1.com Were I got this information...Courtesy of MRTee12 ;)
