Parental Consent Notarization. Lawyer or Civil Registrars?

Hello, Has anyone here gotten their parental consent that is required for the CFO for younger fiances from a lawyer?

Is it really required to get it notarized with the civil registrars or not?

Your fiancee can get Parental Consent form from the Local Civil Registrar in her area. It will have to be notarized by a Notarized Public. Usually there are notarial services within the municipal or city hall so it's not hard to find one.

Word the parental consent to also give their permission to leave the Philippines.

Is this a form that her parents can pickup at the Registrar or do I print it out for them and have them take it to a notary?

Can I use the form from the link above that was copy and pasted and add "and give my daughter permission to immigrate to the US"?

Yes it's definitely required to have one.. better to be safe than sorry.. when I had my seminar I was asked by the counselor if I brought parental consent signed by my parents/guardian.. I have it notarized by attorney at our city hall.. I was 21 btw..

My fiancee, 18 at the time went to the city hall in town. Used the one we downloaded off of VJ. It was notarized there. No need for lawyers.... They didn't even look at my wifes, they got her on something else to make her come back the next day and thats their mission. She didn't have a landline phone no. In the states for me and thry wanted phone numbers of friends/relatives in her baranguay and she left her phone at the hotel... They were really interested in scaring the whole group about Swingers, but don't get me started. Hope it's better now with the new process.

27 January 2012: Mailed I-129F

03 February 2012: NOA1( e-mail & Text)

03 February 2012: Check Cashed

NO RFE'S

22 June 2012 : NOA2 (e-mail & Text)

16 July 2012: Manila Case Number(by phone)

17 July 2012: Interview paid at BPI

19 July 2012: Set interview for Mid-Aug

23-24 July 2012: Medical St. Lukes(passed)

24 July 2012: CFO Seminar(had to go next morning for landline #)- PASSED

02 Aug 2012: Received e-mail from USEM our case is there.

15 Aug 2012: Interview at USEM - APPROVED

13 SEP 2012: POE Minneapolis, MN

27 OCT 2012: Married

19 NOV 2012: AOS package sent

05 DEC 2012: NOA's I-765, I-131, I-485

14 DEC 2012: Biometrics appointment finished(Walk-in..Was scheduled Jan 04 2013)

02 FEB 2013: I-131 and I-765 Approved

07 FEB 2013: USPS Picked up the combo-card

11 FEB 2013: Received Combo-card

21 FEB 2013: Transit Visa picked up in Chicago for Japan

Thank you for your answer. I know the parental consent is required to have, but what im really asking is if the notarization can be performed by an attorney or is the notoarization required to be done by the civil registrars.

My fiancee wants to get it done with an attorney that she is already familiar with. So from what im gathering is that she can print out the consent from the link provided here or pick up the consent from the civil registrars and then just get it notarized from an attorney and then present that form to the CFO.

Yes just have her go to the nearest municipal hall at her area.. We have an attorney/lawyer at the city hall.. I just paid 200php for mine to have it notarized.. you could use that pattern above. At our municipal hall they have form (parental consent) I don't know if other municipal hall has form too.. I brought mine anyway (not from our city hall)

If she knows an attorney then it's fine.. as long she won't pay too much!! Have her parents/guardian sign the form as well.. in my case I brought my mother w me and we both signed the form.
