I have looked at both the guides and the example forms but still have a few questions regarding the DS forms.

On form DS – 156, nonimmigrant visa application, it has a place to glue or staple a passport picture should we add the picture or does the embassy do that? On question 34 it asks the name and relationship of persons traveling with you. Is that just for people who are applying for a visa as well or should I add my name (US citizen – petitioner) since I will be traveling back with my fiancé.

Form DS – 230

question 11 asks where he would like to receive the green card. Can he have it sent to a PO box?

question 22 & 23 asks for his father’s current address & if deceased the year of his death. Since his father is deceased should I write none in the address box and then the deceased year in the next box or under the address should I write deceased?

Questions 14 – 18 ask about his spouse’s information. Since he is divorced should I write none? The other forms have asked about his ex wife’s information but I am not sure if I should add her information since she is his ex and it doesn’t give a space for termination of marriage.

question 33 asks to list all educational institutions attended. How far back should we go? My fiancé didn’t attend college he went into the marines after high school. Should I add the high school he attended or is this just for colleges?

I'm not sure how the embassy in Chile will want it, but I was advised by someone that went through recently at the embassy in Colombia to just glue the photo to the form, not to use staples.

You can put NONE for address, no need to put deceased. My fiancee doesn't know anything about her father, so I have to write UNKNOWN on all of those lines.

For spouse, you would write NONE (since he does not have a spouse).

For the educational question, we went all the way back to her primary school. Not sure if they need that far back or not, and actually I am curious to know the answer to this too.
