Do I have to bring Additional Questionnaire Form in interview?

I know Additional Questionnaire Form is not applicable for all country, but as it mentioned the website of dhaka embassy I am really concern about it. Do I have to bring fill up Additional Questionnaire Form in interview? Why the form is not available in the website?

I have my interview in few days, please help.

In my case, I contacted them directly once they received the case and they emailed me the Questionnaire. They actually mail it as well, however, I never received the mailed packet.

The Embassy was very responsive with email contacts. So, Just send them an email they will send it to you if need it.

Thank you very much for the quick response. Today I email the embassy about Additional Questionnaire Form. I do not know how long they will take time to send it. Please send me the emailed copy of the Additional Questionnaire Form you received from them, then I can be prepared with the fill up form asap.
