Hello, I am the US spouse and I filled out both forms...One for me and one for my husband...I just signed my name about the both of both forms and we were approved in 9 months...I told you this so you will know I know what im doing!

Your family name is your married last name...

Your parents family name is there last names at birth..

You do not put none, the instructions said to put N/A when it doesn't apply to you...I put N/A for my non us spouse in the line for Social Security #

For current husband or spouse, if you have never been married put N/A if divorce then put your ex husband name there. Yes give your maiden name

Yes put present address first and make sure your dates add up to five years. if u have lived in the same address all your life then u should have one entry.

you have to put all employment for the past five years, even if u are not working now put your last job first and go downward

This form is submitted in connection with an application for I-130 is the answer if u are filing for a spouse visa

Hope this helps!God Bless

Correct

* i d'ont have file number or Us social number i put ,"NONE " ?

Yes

* 4th line: current husband or wife (if no so state)family name (for wife give maiden name ): should i put here my husband family name ?

Yes

* Applicant's residence last five years , list present adress first :

street and number ......city..... country.....from (month/year) to (month /year )present time :I never reside outside my country i have the same adress since my birth :) what should i put in this location ?

Since you have lived at the same address since birth, you will put the month and year you were born on the from column and put "present time" on the to column.

* Applicant employement last five years list present employement first : at the present time i'm student in the university but i have already employment in th past, how to answer this question?

You write any employers you have had during the timeframe that is being asked for. If you have not been employed during the timeframe, so state.

* this form is submitted in connection with an application for ,what my answer should be?Status as Permanent Residen ? or other/ I-130 application for spouse"

Tick "other" and write "Form I-130"

you can search a sample of G325A on google. Just search Sample G325A and you can view how the form is filled. Almost all forms' samples including I-130, G325A, I-864 & DS-230 are available online and you can take a look at how each of them are filled
