Some get Rfe's some Don't and sending the DS-3025 saying incomplete is a hope for the best option. It's totally up to the person if they want to get the I-693 and yes many get ripped off by the CS charging hundreds of dollars.

My DS-3025 was in that case, incomplete but eligible for a blanket waiver. As explained above, my vaccination requirements were all good, all others were "not age appropriate", that is why the doctor at my K-1 medical ticked that result. My vaccination history was not considered complete because I had some "not age appropriate vaccines". There was nothing more I could do.

I sent my DS-3025 with my AOS package, totally confident that it was the right thing to do, got no RFE, no problem at interview.

People who get RFE are the ones who think their vaccination requirements were complete when it's not, or people whom all medical documents were not forwarded properly at POE (which can happen to anybody, even the ones sending Part I of the I-693 + the vaccination page).

The OP just need to know if his wife met the vaccination requirements at the time of her K-1 medical...

Complete would mean every shot on the list was given. Some are not medically appropriate for adults. Varicella isn't appropriate if you already had chickenpox. MMR is not appropriate if pregnant. Anything not medically appropriate is eligible for a blanket waiver. That's a waiver that does not require an application form. It is one the adjudicator can grant based on the DS-3025 having the appropriate reasons for waiver checked like --not age appropriate, insufficient time interval, or not flu season. If the panel physician doesn't fill it out properly (marking waiver columns correctly) then it could be rejected. A couple of years ago, some Canadians were getting RFEs because the panel physician wasn't marking the results section and signing. I believe that's what the I-693 instructions (page 4) means when they say "properly completed", ie. All required shots noted, waivers marked, results section filled in, signed, dated. There's a lot of components to getting the DS-3025 accepted. Some people are mislead on VJ to think it's all about what the results section says. One camp says if it says blanket waiver it's fine. Another camp say Complete must be checked. Those are only part of the story. Thus RFEs because something wasn't right on the form.

This is from the Technical Instructions for Panel Physicians: (my bold font)

Completion of a vaccine series is not required to conclude the medical examination, because such a requirement would require multiple visits to a panel physician and could lead to unnecessary delay in the immigration process. If any of the boxes under the "Not Medically Appropriate" heading was checked, the "Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate (as indicated above)" box must also be checked.
This box will probably always be checked because some vaccines may not be age appropriate for the applicant.

Yeah the check was missing from my husbands and we didn't want to take a chance of getting an RFE.. We were happy when the CS confirmed it was incomplete. I'm sure some countries complete those correctly all the time, in our case it wasn't complete.

Spending the $77 was good money spent :)

I sent in the DS-3025 with my AOS application. I heard nothing through the whole process, but at my interview I got an RFE for the I-693 vaccination section only. The IO said that rarely do the doctors doing the medical exam fill it out correctly. I had all of the vaccines required but I just needed it transcribed onto an I-693 form. I went to a civil surgeon on Tuesday to get it done but then he said that I needed a full medical exam again. I said no way and that the IO said to make sure that the civil surgeon was told he only needed to complete the vaccination section. So after going home angry I rang another 4 civil surgeons on the Nevada list before I found one that would transcribe the DS-3025 to an I-693 only. All of the rest wanted me to do another medical. So I have an appointment with him for the 13th of November, and then I will submit it. The IO said that my green card woudl be approved after he receives it.

So long story short....to be safe get the DS-3025 transcribed before sending in your AOS application. I know others have been approved with out it but I wasn't and I did not get REF'ed until my interview.

I agree, we went to CS in USA just to avoid any RFE and additional hassles. It was worth a few bucks to not worry about it at this point. (personal opinion)
