What to put down for city and Country on G35A?

1.)

I live in the city of Elmhurst, in the county of Queens, in the state of New York, in the country of the USA.

Elmhurst, USA makes no sense though.

Should I just put New York City, USA?

2.)

I'm not sure what to put down for my father's city and country of residence. He may have USA residency, but he has been live in the Dominican Republic for like the last 3 years. Should I put Dominican Republic?

Put what the form asks for, city and country of birth, and city and country where your father currently resides.

The borough of Queens is usually called Queens, NY. You can call it NYC if you want. The county is also Queens, but that's not why it is Queens. Brooklyn is in King's county, for instance.

The place of residence is where is he currently sleeps at night. Sounds like DR.

City is technically "Elmhurst", but I can't put Elmhurst, USA because there's also a Elmhurst in Illinois

I'm going to just put NYC, USA

As for my father, he sleeps in Santo Domingo. And he's been there or awhile. I just didn't want to mess up any dealings he has where he's getting a pension or whatever.

I just spoke with my father on the phone and he told me to put down his city and country of residence as "New York, USA", which is technically true because one has to be a resident here in order to receive pension and Social Security which he does receive. I hope it won't be an issue.

It can be; it's a lie. they are not asking for legal residence. They are asking for residence.

Is the city technically "Elmhurst" on your birth certificate? If so, that is what you write under "City and Country of Birth". That is exactly what the form is requesting. There are 'duplicate' cities throughout the states. I live in one now! That doesn't matter as far as what the form is requesting.

Well, he's been there for nearly a year now, but before that he was in NYC for about a year. And he may just decide to come back to NYC anytime.

What's the difference between residence and legal residence? They are asking for his residence on a document that has legal bearing, right?
