Do you have a copy of your vaccination record from your K-1 medical? If yes, that's good to include a copy of it.

Also, the check is not listed in your enclosed documents. I'm sure you include it inside, but just to make sure you think of it.

Which supporting evidences do you have for the I-864?

It's optional, but make a copy of your certified copy of your marriage certificate, and include it with your EAD/AP applications, just in case, so it shows your name change (if you did change name)

And yes, separe each application but include them all in the same envelope. Send to the Chicago lockbox (with delivery with confirmation).

I do have the vaccination record from cuba but they are not in good condition so that's why im not sure to send it ... yeah i have to put the check !!!!!!!!

i send one certified copy of the marrige certificate ... but hi is the beneficiary so i guess is not problem since he didnt change the name ...
