Hello to all, i have a question regarding page 3 of the CRBA checklist, me being the US citizen, what do i write? Do i write the places ive lived? The states ive drove through? The states ive flown over? Im confused? Because i dont know the dates of the times ive driven through places. And for when it says dates present outside the US do i write the dates i was in other countries? Thank you to anyone who helps.

You write what it says, PLEASE LIST DOWN BELOW ALL THE DATES WHEN YOU WERE PHYSICALLY PRESENT

INSIDE AND OUTSIDE THE UNITED STATES. They don't need to know where you were everyone minute of your life while you were inside the U.S. just times you were inside the U.S. and times you were outside. So assuming you were born inside the US you would write for the first entry ##/##/#### (date of birth) ##/##/#### (date you left the US for the first time. The state you lived in is not that important, just the fact that you were inside the US.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

I have no idea what "page 3 of the CRBA checklist" is. Nor should I have to locate it if you're the one that wants an answer. Post it and you'll get more answers.

As to the question "And for when it says dates present outside the US do i write the dates i was in other countries?" I think it's pretty dang obvious what it's asking but just in case, I bolded the important part.

Honestly, and i'm trying not to be rude when I say this, if such a truly simple question is tripping you up, I suggest you get someone to help you. Whether it be an attorney or.. whatever.

It is very rude and unnecessary, if you have no advice 'Vanessa&Tony' or your unable/unknowable in the questions being ask don't comment. This includes the obvious.

For page (3 see link below) you only list the dates you were physically present inside the US. So the date you was born to the date you left that state, if you never left simple enough. If you moved list the dates from-to and the state you moved and so on. The same go's for if you lived outside the US on the CRBA checklist.

In reading your past posts you seem to be struggling in comprehending the CRBA procedures, You really need to read the link: http://manila.usembassy.gov/service/citizenship.html this link has all the answers you need. The CRBA @ USEM is a straight forward process. All the answers for your questions for the CRBA and DS-11 are on the link above and on the DS-11 instruction page of the passport form

Yes she needs a US passport if she want's to inter the US. All you need to do is follow the instruction on the DS-11 and the TWO PARENT CONSENT FORM

Unsigned, completed Application for a U.S. passport (Form DS-11). Click here for Form DS-11 PDF format. Click here for Form DS-11 wizard (if completing form electronically or online). Complete but do not sign this form until you are before the Consular Officer. For children under the age of 16, both parents/legal guardians are required to come to the Embassy in person to accompany the child. Please click here for the Two Parent Consent Requirement for Passport Issuance to Minors under the Age of 16. Passport Fee. The passport fee for a child under 16 is $105; for those 16 and over, the fee is $135.

Statement of Consent Form (Form DS-3053) signed by the absent parent if one of the parents of the child under the age of 16 cannot come to the Embassy (click here to download the form). The form must be notarized by a U.S. commissioned notary. (The Statement of Consent document is valid in the passport application process for 90 days after the date of notarization.)<LI>If both parents of the child under the age of 16 will not be present and only a guardian will be signing on the passport application of the child, submit guardianship affidavit or special power of attorney (SPA) signed and executed by both parents along with signed and notarized Statement of Consent form (Form DS-3053) from each parent.

Honestly, most of the questions that wouldn't apply to an infant are qualified by "if applicable" or "if over age 16" and so on, and the rest (phone number, address, etc.) you can just enter your info there since it should be the same. It's not a very big deal, only 2 pages, just fill out each section the best that you can, leave blank the sections that say if applicable or if over 16. I did the DS-11 for my son when he was a newborn, and went through the fun of obtaining my husband's consent from a separate country, so I know it can be aggravating, but once you've got that passport, baby's good to go for 5 more years so just jump through those hoops!

Most people who have posted info in this thread are very familiar with CRBAs and the forms involved and don't necessarily require a link to the forms referenced. Long story short, we have a complicated case. We've been at this for nearly 5 years. You can read our story here. I highly recommend our attorney Laurel Scott, as well as attorneys Laura Fernandez and Lizz Cannon .

Filed I-130 via CSC in Feb 2008. Petition approved June 2008. Consular interview in Mexico, Oct 2008, visa denied, INA 212a6cii. We allege improper application of the law in this case.

2012, started over in Seoul: I-130 filed DCF on 7/2, I-130 approved 8/8, Medical at Yonsei Severance 11/20, IR1 appointment in November 2012.
