Hi, part of the vaccines was done in London and varicella was done here in the United States. Instruction for I-693 says we need to fill out part 1 and the vaccine page to be completed by a Civil Surgeon. But @ the end of part1 it says "do not sign or date this form until instructed by a Civil Surgeon". To our surprise, the Civil surgeon refuses claiming we didnt have our vaccines and physical done @ his. Hence, he could only transcribe to all the vaccines we provided and that he has nothing to do with the part1. Though, he handed a sealed envelop to us containing the vaccines transcription. so, how do we do the Part1 which says we must be authorized b4 we could sign and dated it?

So the sealed envelope only contains the vaccination page? How much did he charge you for this half-done work?

Last box of the Part I has to be completed by the civil surgeon! Did he even look at what is asked in Part I?!

And Part I+the vaccination page should be in the sealed envelope.

He charged $50bucks, claiming since we said we only want him to transcribe ONLY. He also said he cant authorize it since we didnt have them done @ his place. he didnt even talk to us directly, we were conversing with him through his secretary. We had our medicals done in London and the Varicella done over here.

He's wrong, but if you can't convince him of it you'll have to find another civil surgeon and be out of pocket for the 50 plus whatever it costs you for the new one. :angry: For what it's worth, we paid $40 for the transcription (brought along the medical records showing appropriate vaccination - the vacs were NOT done at the civil surgeon's office) and he signed Part I no problem. Ridiculous. Find a new civil surgeon that will do it correctly and sign off on page 1 and transcribe ALL your vaccinations onto page 5. I personally would ask for my money back, since he did not fill out the I-693 like he was supposed to. I doubt he will refund you, but I would take the chance if even just to let him know I was angry about being ripped off by him. This is the most frustrating process of adjusting status and I wish the USCIS would streamline and regulate this process for those adjusting from a K-1. I won't hold my breath on that one either though.

You need to call around until you find a CS that can do the I-693 transcription correctly for you. Print out and show them the instructions if you have to. We drove to a CS 2 hours away to have ours done correctly. We were charged $25 for the transcription and $30 for my husband's flu shot. Everyone else near us insisted on a new medical or to charge for one anyway. Be sure and take your DS-3025 from your medical and proof of the varicella the other CS gave you. It is flu season, so you also need to get that too.
