DS-156, additional visits to US

What format should I use for listing my additional visits to the US?

It asks for most recent as Day/Month/Year, and then asks for how long.

Right now I have my 6 most recent visits (these were all within the span of my relationship, I've been two other times when I was little with my family) and I formatted them "month year - month year"

The example form on VJ has no listed visits other than most recent, so I'm not sure ?

Turns out the answer was on the example the whole time, I just couldn't see it.

For the record, the format is "Day Month Year (of arrival), length of stay"

Hi there,

I also have a few questions,

1.I am looking over the DS-156&see that for some of my visits to the USA I put the wrong year. Can I redo the DS-156&resubmit it at the interview?

2.If I need to redo, I need to update for my Christmas visit to see my fiancee,correct?

Hello everyone!

I have a question about the DS 156 form. My Fiancé has a son, but due to personal reasoning with the mother, he has no contact with the son and therefore will not be petitioning for him now or ever. At the bottom of the from it says to bring the son's birth certificate. Is this necessary if the son will not be traveling with him? I assumed since he has no contact, and doesn't even know where the son is located, that it wouldn't be. Plus it's clear on the forms that he isn't bringing him along. Anyone had experience in this situation? Thank you.
