My husband didn't receive a copy of his vaccinations when he went for his Medical Exam for the K1. How do we get his vaccination supplement for the AOS?

This is the only thing we have left. Thanks!

K1 Visa process

January 28, 2006 - Send in I-129F packet to Nebraska!

January 30, 2006 - Check cashed, case received by NCS.

February 2, 2006 - NOA-1 RECEIVED!!!!!!!! (mail) HURRAY!!!!

can you get a hold of the doctor in Peru who did the original medical and ask him/her to send you a copy?

If not then you will have to apply for another one through the CS here but I think it might be a bit expensive to have the titres to prove that he has been vaccinated.... try to get the original one off the doctor in peru...

May be he have to have a new vaccination suplement by a Civil Surgeon here, that's what they ask you for the AOS, so may be you should save time looking for a Civil Surgeon who fills the I-693 and the I-693A formats because you will need them.

i know that normally your husband should have his vaccination country with him so he can use that or just go in and talk to the civil surgean

Our experience: My husband contacted his doctor back home about what he needed. His mom picked it up and mailed it to us, we took it to a CS who then transferred the information (in addition to a tetanus shot he needed) to the I-693A. They placed it in a sealed envelope (do not open it!) and we sent it off with the AOS application.

As well, if you have evidence that you have already received these immunizations (ie. childhood medical records, immunization book, etc.) you can bring those to the civil surgeon and he should be able to transfer the information directly. These would be the same records your panel physician used to determine what vaccinations you did and did not have. The only difficulty would be the language barrier which is why the vaccination supplement is so useful - it is in English. If you have such records, you may need to have them translated.
