Hi VJ users,

I got few questions since I can't really find my answers on the instruction Form I-693 and also here on VJ website. The instructions there are really confusing.

I already filled out everything that needed for AOS Form I-485 except I-693. Instructions on From I-693 say that K1 visa holders don't need to submit this Form if AOS is filed within one year.

What is DS-3025? I've never seen it if it was completed or not when I had my medical done overseas. When the yellow envelope arrived I didn't open it and the officer here in Chicago never gave me any papers back from that yellow envelope. Please tell me how others have a copy of DS-3025.

I really hate to go and spend over $100 for the physician so they can fill out Form I-693. I already called the physician’s office and scheduled my appointment for tomorrow at 8:30am. I guess if I don't find my questions answered I might just go and get my I-693 Form filed out by a physician.

YES!!!! I just found my DS-3025 Form!!! Awesome.

But, on the form it is marked that vaccine history incomplete. :( Also, there is a check mark by "Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate."

Should I just send a copy of my DS-3025 Form together with my big packet of AOS?? I hope this will not cause in the delay of my Green Card :) Hey, and I guess I can sleep in tomorrow instead of going and be spending money on those expensive physicians. This AOS already costs a lot of money.

me too is the same happend whit me and i came to usa whit visa fiance and when i wana completed my adjestement of status i make again the test blood and everythings

i was show theme my liste vaccination i was bring whit me but they say you need again to have other vaccination and everythings to send whit your aos and i payed 260 dolars moor

If your overseas medical has been done within a year of filing your AOS, NO medical exam is required, or blood tests, unless you opt for the blood test to check to see if you have had a disease (chicken pox).

You will have to check that on your DS-3025 you have had ALL the required vaccinations (http://www.visajourney.com/forums/topic/333433-official-vaccines-list-for-aos-october-2011/). If you do not have the required vaccinations, you will have to get the vaccinations and have a civil surgeon here fill out the vaccination portion of the I-693, shop around because the prices very and some insist on doing another full medical, which is incorrect! http://www.visajourney.com/forums/topic/231026-civil-surgeons-who-dont-rip-you-off/

We did not have a copy of the DS-3025, so we had to go the I-693 route, which was a pain in the a##. My understanding was that if your DS-3025 had the required vaccinations, that a copy of the DS-3025 was the only thing needed to send in with your AOS packet. Now I have read here where people have gotten an RFE without a transcribed I-693 with their DS-3025, maybe someone can answer this a bit better that has some first hand knowledge.

A big tip to all that read this, get the vaccinations that are required for AOS done at the overseas medical (Kiev for RUB's) and get a copy of the DS-3025.

•MMR

•Td or Tdap or DT or DTP or DtaP (One no longer than 10 years ago, ie.have a booster)

•Varicella or VH written by it if you had chickenpox

•Influenza **see note below because you are most likely okay without it.[/size][/size][/b]

OK, here is what I don't get there. I do have a check mark on a Form DS-3025 where it says Td and a bunch of dates. Another check mark I have where it says Hepatitis B and with some dates there as well. Everything else is check marked as "Not Age Appropriate". And there is NO check mark in the MMR, Measles and Mumps and there are no dates but there is a check mark in the "Not Routinely Available". So, should I go and see the civil surgeon? I did have my physical done in the past two month which is less then a year old.

Yes. You need to see a civil surgeon and get the missing MMR. Did you get varicella, or was it marked with VH because you had the chickenpox? Take your DS-3025 with you so the civil surgeon can see what vaccinations you are needing and can transcribe them onto the I-693 form. It is flu season now, so you will also need a flu shot.

That is de price, it goes from 230 to 320...

2012.

July 23rd: Arrived USA - POE JFK

August 4th: Marriage.

Week 20nd to 26th: Convinced by wife to stay in USA.

October 23rd: Package Filed to Chicago Lockbox

October 29th: NOA's

November 6th: NOA's received

November 12th: Received Biometric Appointment

November 20: Took Fingerprints and photo at Boston office.

December 13th: Interview Letter received and scheduled to January 9th, 2013.

January 9th, 2013: AOS Approved!

January 19th, 2013: Resident Card Received!! :D :D

Looking forward for my GreenCard to arrive!. :D :D

Was it the cheapest around or did you go to the first one?
