Thought I'd post a copy of what the medical questionnaire looks like for the Knightsbridge Dr's.

Thanks for posting that!

I'm surprised they ask for Visa Appointment Date. It seems that most often, people have their medical before an interview date is scheduled.

When does one get this questionnaire? is it with the pack 3 or actually on the da of the medical, basically do we have time to fill it in or does it need filling in on the day of the medical? Im concerned about what to put I have had depression but its all mainly anxiety attacks and I am on medication for that and have been for nearly 3 years or so. Would that be something to worry about at the medical??

27 Feb. 2006: K1 petition sent to Nebraska Service Centre

2 March 2006: NOA1 dated (received in mail on 9 March 2006)

4 May 2006: NOA2!!!!! (DAY 66)

8 May 2006: File Sent to NVC (as stated on email dated 9 May)

24 May 2006: NVC sends our file to London Embassy

8 June 2006: Packet 3 arrives (DAY 101)

25 August 2006: Pack 3 sent to London Embassy registered post

29 August 2006: London Embassy receives my Pack 3 - 9/9/06 pack 4 arrives

SEPT 2006:APPLICATION PUT ON HOLD - WE HAD A FALLING OUT.

MAY 2007:RESUMED THE VISA APPLICATION

6 June 2007: Medical @ 2.30pm - need to get anew GP letter and psychiatric evaluation

7 June 2007: INTERVIEW @ 10am - waiting for new police record letter

3 July 2007: APPROVED/Visa arrives

8/18/05 received NOA!!

9/9/05 I-130 APPROVED!!

2/13/06 CASE COMPLETE per NVC - got letter on 2/25/06

3/28/06 - INTERVIEW SCHEDULED FOR 5/2/06 @ 10:30 A.M.

Medical scheduled for 4/10/06 - everything went great!!

5/2/06 - INTERVIEW - APPROVED !!

5/20/06 - Crash came home to NY for good!!

Lina - From what I have read on this board, I think depression, anxiety and other similar maladies are addressed by some questions from the examining doc. If you are better, they seem satisfied with that. There have been some situations of self-harm reported here, and the docs do question that a bit more rigorously - again I have seen visas granted in those cases as well. You need to be prepared to answer honestly and be able to tell them what meds you are on. Otherwise, from what I've seen on this board, I don't think you need to worry.

They started doing that when the procedure for the medical was changed. I think they wanted to make sure they scheduled the people with interviews BEFORE those without to make sure they could get them done in time.

Visa appointment date is to ensure they are able to get the results from the medical to the Consulate in time for the interview. They told me that even though the Consulate says allow 2-3 days between appointment and interview, they really need 5! Seems like they have more work than they can handle.
