Hi everyone..

For those applicants who submitted their AOS recently, I need your help guys.. I am from Philippines and arrived here in Colorado last month (Sept. 2) w k1 visa.. got married after 10 days and we will file my AOS in 2 weeks.. I want to know how important to get my medical transcribed by a civil surgeon (I-693).. I thought it's optional but some says they are getting an RFE because they haven't submit the I-693 form.. I had my medical in Saint Lukes on July 24-25.. For sure we don't want to get RFE..

I-485, g325a, I-864, I-765, I-131 and g-1145.. these are the forms I would submit.. in which lockbox should I send these?

Please vj'ers give us some insight..

Take your medical stuff you have to a local CS and get it finished, no big deal. if you are on insurance it will cover part of it, not the government paperwork.

- Aug 31st, 2011 - Mailed I-129F package - May 29th, 2012 - Mailed AOS/EAD/AP package

- Apr 13th, 2012 - Visa received - Aug 24th, 2012 - Green Card received

- May 19th, 2014 - Mailed ROC package to CSC

- Aug 8th, 2014 - Green Card received

N-400 Timeline

- Dec 29th, 2021 - Filed online. Got notice that biometrics will be reused.

- Now waiting...

Take your medical stuff you have to a local CS and get it finished, no big deal. if you are on insurance it will cover part of it, not the government paperwork.

My husband just added me to his medical insurance last week..

I-693

According to the I-693 instructions, an I-693 is not required for K-1'ers if they are filing their AOS within one year, they don't have a Class A medical condition, and their DS-3025 has been filled out correctly.

Also, those who have already received the age-appropriate vaccinations, and where there hasn't been enough time between a first vaccination and it's second dose, will be waived by the USCIS.

However, some people who are correctly submitting only the DS-3025 are getting an RFE. So, better safe than sorry...have a Civil Surgeon transcribe your DS-3025 onto an I-693.

When I read the original post and I saw that the op had her exam at St. lukes, I thought she meant the St. Lukes in Pennsylvania, if that was the case then she would need to file the I-693. Upon further review, I see that there is a St. Lukes in the Phillipines also. If her exam was done there, then what you say is correct.

When you file the AOS from a K-1 visa, you just need to provide a confirmation from a Civil Surgeon in the US that you have completed all of the required and appropriate vaccinations. You do this by having a Civil Surgeon transcribe the information from the sources that you have to the correct pages of the I-693. If you completed all of the vaccinations before entering the US, you could just provide a copy of the form given to you by the Panel Physician stating so in with your AOS application. You may or may not get an RFE for the vaccination history. If you do not have a copy of the form or if you had to complete your vaccinations here in the US, then you will need to get the Civil Surgeon to complete the appropriate pages on the I-693. You do not need to re-do a full medical if the medical is less than one year old - unless you are told by USCIS that you need to re-do the medical. (There are a number of reasons why that might happen).

So, take your vaccination records and get your vaccination history transcribed by a Civil Surgeon in the US to the I-693 (he/she will have the form) and then submit that with the AOS package. You can phone around to different Civil Surgeons and find the one who will give you the best price. If he won't do it unless he gives you a new physical, then find a different Civil Surgeon.

So all those forms I mentioned above are going to one lockbox which is in Chicago?

Thank you so much! Yes we did.. at first call civil surgeon wanted to re-do my physical exam. We tried to call different cs, found one which they know what to do with my ds2035 onto an I-693 :)
