Hi Everyone,

I just got a letter for a request for additional evidence. It states "The US Department of State medical forms DS-2053, DS-3024, DS-2045 and DS-3026 were not forwarded by the Department of State."

I called the immigration office and they said that I need to take another medical.

I took my medical in Montreal April 2011 and interview was on Aug 2011. I entered USA Oct 15, 2011, was married Jan 2012, and filed Jan 2012.

Can anyone explain to me why I'm being asked to take another medical?

It sounds like your medical got lost! Sorry, can't offer any advice beyond that, though maybe someone else will chime in as losing medical exams seems to be common...

And if you aren't able to get another copy, you will need to redo the medical here. It sucks, I know, but individuals have had their AOS denied for not providing a second medical when asked, responding that they already did it back in Canada and it should still be valid. It is, unfortunately, a very common problem - the medical getting separated from the file - and one that shouldn't happen. The POE should be able to keep the packages together when they open them up at entry and forward the whole package. Somehow, they sometimes don't.

Good luck - and hopefully the doctor's office in Montreal did keep a copy.

Hmm, you could also try contacting the Consulate in Montreal and ask them if they still have those listed documents. They were supposed to be in the brown envelope you submitted at the POE, but in the off chance that they were the ones who screwed up instead of the border, it is worth following up.

There is an FAQ on Medicals in the pinned topic at the top of the AOS Process forum... where I am moving your thread to.... which discusses this issue. Briefly, there are two files... a temporary and a main file. Sometimes the medical documents are in the other file. Contact your Congressperson to make an inquiry and get things back on track. There is absolutely no reason for you to be paying for a new medical when your current one is valid. I have also seen where some Canadians were able to have a copy of those documents re-sent with no problem at all, so definitely contact the Panel Physician also.

I got the same letter too. I am asked to provide them divorce paper of mine and as well as my husband's. in addition, they said the same things, all medical forms are not forwarded to them. I also need to submit form I-693 with medical and vaccination records.

I had my medical exam in Toronto last November and POE before Christmas and but I don't know where the I was frustrated yellow envelop is now. I was frustrated at first, i called the department of state and they told me that they cannot track the document as the case is processed.

The good news is. I went to the civil surgeon office yesterday intended to have my vaccination records transcribed, but when they read the letter they said that i need a medical exam. So I had my blood test there, and will get the result and all the paper work done tomorrow. it only cost me $150 which i think worth it if it works.

You can do it here too. Just call them and ask what they can do and how much it will cost. some civil surgeons charge a big copay.

The CBP agent at our POE (LAX) was kind enough to tell us that they lose the hand carry packet "all the time", nice. My wife was not given anything following her exam. I e-mailed the Hospital in Bangkok and they sent me certified copies of all the forms the next day, no charge! Trying send an e-mail and asking. I believe you will need to submit a copy of your bio page of your passport and birth certificate.

I went through the whole 'lost medical' thing. It sucks. A lot. Because it's completely out of your control.

Hopefully you get something figured out, otherwise another medical is needed.

Frustrating that they are so disorganized but it's just faster to go have the new medical done then try to back track and find it or get a copy from the Canadian physician - in my opinion.
