As I previously posted, my wife was not given a copy of her DS-3025 at the exam. And of course, I have no idea what was inside the packet we hand carried to our POE. I was successful, at Karee's suggestion, of getting a complete set of records regarding the exam from the Hospital where it took place. I wanted to make sure we had these so as to avoid another exam here with a CS, if possible.

Here's my question: On form DS-2054 (3) the doctor ticked "Vaccine history incomplete, requesting waiver" and then ticked "Blanket waiver" The same boxes were ticked on the DS-3025. My wfie did get the Td and MMR vaccines. What is the signifigane of a "Blanket waiver"? Thanks.

Kind of. I guess some vaccinations she was not "age appropriate" for. But what I'm trying to figure out is based on the "Blanket waiver" will she need another exam here by a CS? She had her exam in Bangkok on 8/14/12. Thanks.

Not necessarily. I was in the same case (incomplete, eligible for a blanket waiver). You just need to know why the doctor ticked it. In my case, it was because I had some age not appropriate vaccines, all other vaccines requirements were met. Nothing I could do against that, so I sent a copy of the DS-3025, got no RFE, no question asked during the AOS interview. Other medical infos (results of blood test, x-rays...) were inside the envelope given at POE. It is supposed to be forward to the DOS/USCIS then. You are supposed to see/have them (scelled envelope...).

If your K-1 medical was done less than a year ago, and that your vaccination requirements were all met, you just have to send a copy of the DS-3025. No need to see a civil surgeon. If some vaccines are missing in your DS-3025, you need to go see a civil surgeon, have the vaccines done, and have him complete part I of the I-693 plus the vaccination page. All should be then put in a scelled envelope and included in your AOS package.

Thanks. She got the Td and MMR. The other vaccines were ticked "Not Age Appropriate" and that the applicant may be eligible for a blanket waiver. Given that the exam was done 5 weeks ago and we'll be filing for our AOS in 2 weeks, I reckon she won't need another exam and an I-693 and we shouldn't get a RFE.

It shouldn't, no. Be aware that sometimes, the medical docs in the scelled envelope given at POE are not properly transferred to the DOS/USCIS. Nothing you can do against that for now, but it happened to some folks here on VJ. If you're one of those unlucky ones, you will receive a RFE for medical docs lost (not your fault...) and so will have to see a civil surgeon and redo a medical. Cross your fingers so it won't happen to you. :) It's still good to be aware of, but for now, just sending a copy of your DS-3025 is the thing to do if you want to follow the USCIS instructions properly (I-693 instructions form if you want to go take a look at it).

I have read that. In fact, the CBP officer was kind enough to tell us that they lose the packet we brought to the POE all the time. The hospital where my wife had her exam was kind enough to give me copies of all of the documents completed during the exam, DS-2054, DS-3030, DS-3025 and DS- 3026 which I will be submitting along with our I-485 AOS petition. Thanks.

Khwaidee, my wife is also from Thailand and I was also faced with this question when I applied for her AOS. With the advice and experience of a few other people here on VJ I just sent in the DS-3025 and did not get the exam from the civil surgeon and we had no problems.

3-5-2011: I-129F Sent

3-9-2011: NOA1

6-24-2011: NOA2

6-30-2011: NOA2 Hard Copy

7-19-2011: Received Case Number from NVC

08-08-2011: Sent Packet 3 to Embassy

09-12-2011: Received Packet 4 Instructions

09-28-2011: Interview: Approved
