My Fiance is trying to update her Vaccinations so we can send in our AOS Paperwork including the I-163 Exam that she took in Guangzhou.

My question is, wasn't she supposed to recieve a copy of her I-693? So she can show it to the CIVIL SURGEON to complete and sign off? As far as I can tell , she has all of her Vaccinations now up todate from looking at her copy of her Vaccination Worksheet. She did receive her DISC of her xray. I have read she does not need to take another exam now she is in America because she was a K-1 Visa with a previous exam from her Interview.

I think you mean the DS-3025 but I could be wrong. My fiancee was not given a copy of her's either. I've heard it is in the sealed packet you hand carry to the U.S. Just to be safe I e-mailed the hospital where the exam took place and they gave me a doctor's report as to the vaccinations given (Td, MMR) I've asked if they kept a copy of the DS-3025 but I haven't heard back yet.

We have copies of the DS-3025 Vaccination Worksheet, but not the Exam I-693 that was performed by the CIVIL SURGEON in China at her Interview.

We also have what appears as the Original DS-3025 worksheets in small white sealed envelopes. But have no idea what to do with them.

I would check on the AOS forum but I believe those are filed with your I-485 AOS. Since my fiancee was given nothing I am hoping it was in what carried!

If you have a DS-3025 from the medical from overseas you need to send a copy of it with your AOS packet. Now depending on what the DS-3025 says will determine if you need an I-693 done by a C/S here in the states. I am not sure of the wording on the DS-3025 (my wife did not have one) but all of the appropriated vaccinations need to be completed--and stated on the DS-3025. I do not have a list of the required vaccinations handy, but you can look them up on V/J :thumbs: If she does not have all of the required vaccinations, the I693 will be needed in addition to the copy of the DS-3025.

The exams overseas are done by panel physicians. The ones in the US are civil surgeons. The panel physicians do not fill out an I-693. The medical forms from the K-1 medical were in the sealed envelope she handed over at her POE.

If she had all the required vaccinations marked off on her DS-3025, then just go ahead and send a copy of that. If she is missing any, then she needs to see a civil surgeon in the US to get the missing shots and have page 1 and the vaccinations page of the I-693 completed. She was not supposed to "receive a copy of the I-693" in China. The I-693 is not something done overseas, but in the US by a civil surgeon. Now, following the I-693 instructions form, she may not need to have the I-693 completed.

What does her DS-3025 say at the bottom of it? (which case is ticked?) Does she have some missing vaccines on it?

Was her K-1 medical done less than a year ago?

- Aug 31st, 2011 - Mailed I-129F package - May 29th, 2012 - Mailed AOS/EAD/AP package

- Apr 13th, 2012 - Visa received - Aug 24th, 2012 - Green Card received

- Aug 8th, 2014 - Green Card received

- Dec 29th, 2021 - Filed online. Got notice that biometrics will be reused.

I just finished my AOS recently, and I only attached the copy of "DS-3025,Vaccination Documentation Worksheet" to USCIS, as my understanding is I have got shots I need in China on July 30,2012, and I have to wait for three months to get second ones, but now only less than two months; besides, I have no flu shot in China, as it is not flu season at that time, and now, September is not flu season in US either, so I think this document is enough for my AOS. I definately will get more shots later and I will take new documents for interview in several months later. This is what I handle AOS.
