phoenix arizona relliaable civil surgeon

anyone here in , phoenix arizona who can give relliable surgeon , that would not ripped off pocket , for too much charges,, coz i tried to call dr. renuka, they only do paper works for refugees. vaccines they they do this but doesnt include paper work....can anyone advice me for reasonble surgeon that will fill up i-693 form .. (ledesma.rociel@yahoo.com)...

If you already received all of the age-appropriate vaccinations at St. Lukes, then you don't need the I-693. Instead, you can send in a copy of your DS-3025 (vaccination record from St. Lukes) along with your AOS packet.

well in the ds-3025 , says not age appropriate and most of the vaccines check is under it ,, and some are in insufficient time interval??.,,, is this one desame?.., does this means im complete?..,, thank you so much for the respond

He charged us 40.00 to complete the I-693, plus the cost of vaccinations.

You do not need to get the vaccinations which are marked "not age appropriate." Also, you do not need to get (for AOS purposes) the vaccinations which are marked "insufficient time interval." You are complete.

hi there ... Well I just wanna share that I got charged 20$. Becaused I just got my vaccination records transcribe only. In Philippines they already gave us all this vaccines. And labory test. :-D

Put her on your medical after getting married and use that for everything except the USCIS paperwork.
