My wife had her medical at Bumrungrad Hospital in Bangkok. They gave her 4 vaccinations. We are getting ready to file our AOS and the person that is helping us asked for the DS-3025. Problem is, my wife was never given anything at the Hospital nor the interview. I assumed that the medical documentation was in our packet that we hand carried to our POE. I didn't want to open anything from the hospital for fear it would void the exam.

Question, was the DS-3025 in our hand carried packet? If not, how do I get a copy or does she need to do the whole exam and shots again? Thanks.

Yes, look if you can get a copy (if they saved it, maybe they can send it to you by email?). Some doctors give a copy of it after the medical, but looks like it works in different ways in every country.

You did good by not opening the packet and you did nothing wrong. You were not supposed to open it and take the DS-3025. Your wife should have just asked if she could have got a copy of it after her medical.

Folks, some countries don't give copies of the DS-3025. They put all that information in the sealed package. I was told by other members from other countries to have my husband ask about this copy, but the doctor and everyone in the place told him they don't provide. Then, by other members from the same country was informed that Peru doesn't give a copy of the medical. This is totally oK. The CO at POE should send all that information from the sealed package to immigration.

Your medical info should be complete and the shot record included in the package. That info was given to her at the hospital. This is hand carried to be turned in at the POE. It is later forwarded to the office where the AOS interview willl be held.

I am confused. You say she was given nothing at the hospital. Then you say you hand carried a packet to the POE. Which is it?

I think he meant she was given nothing other than the medical sealed envelop. Some other countries give a copy of the vaccination record.

The I-693 instructions states that even if a new medical is not needed for K-1 who file AOS within a year of their medical, they still need to show that their vaccination requirements were met. That is why some doctors issues a copy of the vaccination record (the original is in the sealed envelop) to the applicant so that they can show proof of it in their AOS package.

I have no idea if not sending anything in the AOS package and knowing that the vaccination requirement were met will still be fine. Theoretically, you do not know which case the doctor ticked on the vaccination record if you don't have a copy...

I see what you mean now. I went thru that same hospital and was given nothing outside of the medical envelope. I went to my local county health clinic to get my follow up shots. When that was done they created shot card for me that I could use at my AOS meeting. USCIS wasnt interested in even seeing it and asked no questions because as you said I did it within the one year window.

She was given the x-ray packet and the report packet which she took to the interview. When I say she was not given anything at the hospital I meant other than what she handed over at the interview. I have read that some people are given a copy of their DS-3025 and that it is required for AOS. Anyway, I am in contact with Bumrungrad's medical record department and have in a request today for a copy of the form that they said they will e-mail to me. Sorry for the confusion, we hand carried the packet to the POE and I wasn't sure if the DS-3025 was in there or not, that was what I wa trying to ascertain. Thanks.

When you apply for AOS you will need a copy of the DS-3025 or an I-693 Part I and the vaccination page filled out by a civil surgeon.

That's what I thought. I am hoping to get a copy of the DS-3025 to avoid a visit to the civil surgeon. A request has been submitted to the hospital so I will see what I get. We get married on October 5 and will file our AOS on October 8 if I can get the DS-3025 in time. Thanks.
