I'm a bit confused at this form. Just a couple questions I have

Does my wife and I have to fill and sign part one of this? My wife is from the Ukraine and I dont know what info is needed.

Generally, the only thing you will have to include beyond the DS-230 Part I and the cover sheet is a simple (i.e., not notarized) photocopy of the biographical information page from your spouse's passport. If your spouse is from Canada, a country in Africa, China, and those two or three other countries, more is generally required. (Again, you should find out from a VJ member whose spouse is from the same country as yours exactly what you have to send in with the DS-230 Part I.) Send all of these things to the NVC address listed on the barcoded DS-230 Part I cover sheet

What type of info from her biographical info do I need? I have copies of her passort with english translation already. Is this enough?As I messed up and when she got the choice of agent she signed her name and now they are sending this to her and then I have to have her send it to me so I can pay the bill. I'm so lost at this point any help would be most greatful. Also is it to late for her to email a change of agent once they already got one? How do you do that since she needs to sign it? In the ukraine she doesnt have much acess to faxes and printers.

Hi Robert,

Your wife is the one who has to fill out this form and sign it (only part I), not you. She can do it using the online form from the Dept. of State website. Then she just needs to print it and sign it. If she can't print it, then she can use the form that was included in her Instruction Package. All they want is a copy of the biographical page of her passport (Name, date of birth, country of birth, etc...), not sure a translation is needed (it doesn't hurt to send it if you have one). Now she needs to add all of this to the barcode sheet and send it to them. She can also send it to you and have you send it to them. Oohhh...and she also needs to write her case number on all the pages she sends to the NVC....on the right upper hand corner!! Since my husband is my agent, I honestly don't remember who they sent the bill to. But we sent the check right away. Maybe it would be best if she sent it all to you and you mail it all to the NVC.

I think she can still change her choice of agent, but I don't see why she should do this now. The document she was going to get is the DS-230 and she already got it. And she'll get mail if something is missing. I think she can do that by e-mail.

Thank you for moving this!

I called them last night and since she is the agent they sent her the bill which is 380$ I believe she said. After I pay the bill they will send her the DS230. They sent the bill October 7th to the Ukraine and tomorrow it will be 3weeks. I don't know if she should just email them a change of agent wait till they recieve it and then I call and request another bill. This has been a nightmare as my ex lawyer was our agent I let him go last july and the NVC kept sending him info and he just threw it away. The original bill was sent in the first of august. I had to call to get a request of change of agent. Then they sent him the bill , then I had to call at end of August to get them to send the bill to me by then they got he Choice of agent and the damage was done. I'm going on 1yr since the time I filed and its put alot of stress on our relationship :(
