DS-230 questions on items #31a, 33 and 35

Hello everyone,

I tried searching through past topics regarding my query, however, I failed to find something similar to my questions. Sorry, if these are already answered questions in the past...

1. Item #31a Person(s) named in #14 and #29 who will accompany you to the US now.

My husband, who is the petitioner, plans on travelling with me when I go back to the US for good. Should I put his name or the question is not applicable to him and I should just put None?

2. Item #33 List educational institutions attended.

Should I include high school also or the college level is enough?

3. Item #35 List dates of previous visits to the US.

Should I write down all locations where I stayed at or just the points of entry/exit? For example, in 2009, I visited relatives and friends in New Jersey, Baltimore, LA and Seattle, should I write down all these? Or should I just put New Jersey (my point of entry) and LA (point of exit).

01/04/2013 - Interview @ 6:15 AM (APPROVED @ around 10:10 AM)
01/10/2013 - Passport and IV packet delivered (in 4 business days)

05/21/2013 - GC received

10/07/2013 - 1st Day @ 1st Job

Oops, just a follow-up on #3, double checking if I understood it correctly. These were my POE during my visits:

travel date 1 -> Newark, New Jersey

travel date 2 -> Detroit, Michigan

travel date 3 -> Seattle, Washington
