QUICK QUESTION!! i am going to my family doctor to get a record of my immunizations, is there a specific form she will need to fill out? or just let her give me a copy of what she has on her records? just got my NOA2 and im confused already!! UGH

No special form. She needs to create a record. She'll know This part of the process is between your doctor and you, not the actual medical. So you'll have to take those records to your medical later on.

I picked up my records from my doctor's, because the clinic where I'm getting my medical exam asked for originals.

Don't panic if you hear from your doctor that your vaccination records were incinerated. That happened to my records and a lot of other Canadians. It's really pathetic, I know!!! But there are ways to get past it. You can get tests to check for past vaccinations and whatnot. If you have any questions feel free to ask here or PM me

if you have had the same doctor since birth your doctor should have adequate records...but if not, you may want to contact the public health dept for your immunization records. judging from your picture, you are younger and the public health dept should have all you records on file (computer). if you have a new doctor they may not know (some doctors in the past did not transfer records appropriately) what vaccinations you have had or not had....in this case I'd ask for a titers test on all vaccinations.....as it turned out I was not immunized for rubella and after my medical in TO, I had to get it.....even though my doctor said I was up to date on all immunization.

word of caution....we are approaching flu season....and if your hoping for an interview prior to spring/summer, you will want to put that on your list of things to do.
