We were wondering whether to send the x-ray that contains the medical examination report (in a sealed envelope) to NVC??

We are applying for a CR1 from Japan and we have been asked to submit the visa applicant's visa application form

along with required documents. Medical examination is one of them and we have a medical examination report along with an xray in a huge envelope(!). I'm worried that it will get damaged along the way to the US from Japan...

No, no. The medical report and the X-ray are brought to the interview, not sent to NVC.
