The Bogota Embassy has requested a copy of my W-2 and I am trying to email it to them. The email system just returns with a response of,

"We cannot respond to any case specific inquiry without the applicant's case number, complete name, and date of birth in all correspondence. You must RESEND your original e-mail with this missing information or we will not be able to respond."

I have sent this email 3 times putting the case number in the subject line. Then I tried putting the case number and name and DOB in the subject line but nothing. It keeps getting rejected. How should I send this email? Our visa was approved but they need my W-2.

Then you will need to follow up with a phone call just to make sure they received the email with the attachment. If not, then ask to see if you can send this via fax.

I've gotten responses back a number of times, using the BGT number in the subject line. As a header for the text of the email, I listed Case Number:, Principal Applicant Name:, Preference Category:, and Foreign State of Chargeability:. Then Dear Consular Officer, whatever I asked them.

It turns out that response is always generated even if you include the appropriate subject. They emailed me stating that they received the document. My wife was given a paper that says approved and to contact DHL but they also gave her another paper that stated a scanned W-2 was required to be submitted. Does this mean that the visa is approved or can they still deny it based on the W-2?
