STEP 2: Notarize Form DS-3053 & Submit Photocopy of the ID (Only with Statement of Consent)

Step 2 only applies if you are submitting Form DS-3053 as a Statement of Consent
. Statements of Special Circumstances do not need to be notarized. If you are a non-applying parent or guardian consenting to the issuance of a passport for a minor child, you must:

  • Sign and date Form DS-3053 in the
    presence
    of a Certified Notary Public
  • Submit a photocopy of the front and back side of your ID with FormDS-3053

NO!

All notarized doc’s from the US citizen must be by a U.S. commissioned notary.

The U.S. citizen parent(s) must present his/her current and expired U.S. passports and a photocopy of each. A notarized copy of the current U.S. passport and its pages is acceptable in lieu of the original. If the U.S. citizen parent was naturalized, he/she must bring the original Certificate of Naturalization and one photocopy.

Please bring photocopies when required. Otherwise, you will be charged for the photocopying fee of $1.00 per page at the Embassy.

Many of the documents require you to submit both an original and a photocopy.

The U.S. citizen parent must bring proof of living in the United States before the child’s birth in order to transmit citizenship. bring the original documents and one photocopy.

This includes his birth certificate, have him send the one with the raised seal from vital records in his state.

Bring the original Marriage Certificate and one photocopy. For marriages conducted in the Philippines, only a marriage certificate from the National Statistics Office (NSO) is acceptable.

Your post is relevant when the child acquires citizenship through CRBA and wishes to travel to the US.

Although the statement of consent is needed for the non applying parent or guardian, when applying for a US Passport (DS-11) to a minor under the age of 16. The context of your post has nothing to do with the CRBA process, (DS-2029) as 'PreciousOne' was specifically referring too.The two are separate.

Can you help me? I just got NOA2 for mi wife. Should I do her process first and after that my daughters. Can my wife alone in El Salvador do the CRBA for my daughter or is it better if I go?

Finally we all three together. We are happy :)

Do your daughters CRBA ASAP it is recommended the US citizen apply, however your wife can execute the application.
