Timeline

We're just starting our AOS status and have read that we're not supposed to fill out a I-693 form because we're K-1. Question is

1. Is this true?

2. The clinical surgeon in Juarez gave my wife a Vaccination Documentation Worksheet with her name/alien number but the physician didn't fill out anything. Just dated and signed his name. She also received a CD with her tb exam. Is this what we're supposed to submit in lieu of the I-693.

Any help/advice would be appreciated especially from those who have entered through Juarez.

Unfortunately, Juarez does not give vaccinations at the K-1 medical and either gives no DS-3025 or a blank one. You need all the vaccinations that are required for AOS. You will need to call around to find a civil surgeon that is willing to do just the vaccinations transcription. You need page 1 and the vaccinations portion of the I-693 filled out. You do not need another full medical, but some will insist on giving it anyway. Search for the thread here about civil surgeons that don't rip you off to see if you can find one listed in your area. Bring any vaccination records you have with you to your appointment.
