I am going to apply for my husband's AOS just after marriage using the marriage certificate copy I will receive at court.

What name I should use maiden or married?

I am thinking of filling forms with my new name but all my supporting docs will have my old name.

I want to know the precautions I need to keep as I will apply for name change in my ssn and will apply for my husband's AOS almost at the same time.

We will stay at our current address in different cities until either one of us gets transferred to other's city. Will it be an issue?Whose address I should use?

I have a permanent home but my spouse will live in a temporary apartment.

You should use your married name if that is the name you intend to have after marriage. USCIS deals with marriages every day, that coupled with your marriage certificate will be enough information for them to realize you changed your name. You need to put your individual addresses where they ask. The I-485 interview will need to take place in the office having jurisdiction over where your husband lives.

AOS for my husband
8/17/10: INTERVIEW DAY (day 123) APPROVED!!

ROC:
5/23/12: Sent out package
2/06/13: APPROVED!
