This seems the better of the two possibilities. There were always be people uncomfortable, but I imagine that there would be fewer women disturbed by a transgendered female in the ladies' room than men disturbed by a transgendered female in the mens' room.

But what about transgendered males?

I used to work with a transgendered male and he used the men's room. He looked so much like a guy that it pobably wasn't that weird.

Serious. And it was more than once - we filled the hole with plaster, and the guy (who was never caught) kept going back there and digging out the filler. Finally we drilled a bigger hole and nailed a steel plate over the whole thing. That seemed to stop him.

The staff have a file folder full of 'questionable types' with photos to call the police on. One guy apparently liked to go up behind female booksellers and 'rub himself' against them.
