This will be her first passport. Do her ID's she uses to apply for her passport need to be changed to my last name? Also which ID's will they accept and I'm assuming the passport must be issued in my last name also? So far she has a student permit and postal ID in my last name and she has a voter's ID in her maiden name still.. We have the NSO marriage cert and birth certs already

all she need to do is to bring your original Marriage contract authenticated from NSO, Birth Certificate from NSO..thats the only thing they need (DFA) to transfer her current last name (husband).

Look on the USEM website at the list of document's needed. It sounds like you're good with what you have. If she has her baptismal cert I would bring that as well, in the Phils it's treated like an I.D. Not sure about the CFO as I have heard conflicting stories on this one depending on where you get the passport at. You will eventually need the CFO for her to leave so might as well get that done in Cebu as well. Good luck and Congrat's.

IMO, better call DFA or log-in to their website for the list of requirements.. DFA will be the one to inform you what documents to secure and bring. (case to case basis) From my brother's experience: First-time applicant, he was asked to present school TOR(Transcript of records), original& photocopy of baptismal cert. If your wife has digitized Government IDs, she may not need to present an NBI clearance..

Original marriage cert from NSO is a must for married women.

To be safe log in to their website. www.dfa.gov.ph. Hope this helps

All your responses were very helpful. Thanks! So it sounds like she should do the CFO seminar now. She'll have to do that eventually anyway. Also ID's in her maiden name (in her case voter ID) are acceptable as long as we also present the NSO marriage certificate. (which we have) Also required is her NSO Birth Certificate and NBI Clearance.. Good to bring are the Baptismal Cerificate and School Transcript.. So it sounds like after getting the CFO cert we'll be ready. Hope I'm not missing anything. :)

If she does the CFO seminar now, how and when does she get the sticker she will need to put on her visa?

she really is required to attend the seminar. ive seen lots of applicants on DFA being refused to process their passport since they dont have the certificate. if she dont have the visa yet she will be issued certificates from CFO. then as soon as she have her passport with visa on it they will just put the sticker on it no need for seminar again.

So I'm assuming as soon as she gets her visa on her passport she can just go to the CFO office with her seminar certificate and they will put the sticker on it and she will be all set. Any other requirements?

Another possibility (since 2009) is to get the CFO sticker at the airport. The location is given in the link below (hours listed 5AM to 12midnite). But some other posts observed that staffing may be spotty? Unfortunately there isn't enough of a track record so far to be 100% sure this will work, but it would be nice not to fight Manila traffic one last time to get to CitiGold, as your wife likely will be at the airport to head to the US (unless direct flight from Mactan). I've seen the desk in that location last December & it was open at 5AM.
