Question On I-134 Mailed From Embassy

Attached to the appointment letter that I received from the Embassy is a form I-134. The revision date on the form is 07/14/06 but the one online at USCIS has revision 05/25/11.

Here's the major difference is that the oath or affrimation section is different. The one sent by the Embassy has a space for a notary public while the one I downloaded from USCIS website does not require a notary signature.

I would love to hear from successful K1 (not IR1/CR1 who uses different forms) applicants and preferably out of Lagos Embassy.

For future searchers: I di some more research specifically as it concerns K1 and Lagos on this site and others. It seems both versions have been accepted at the Lagos Embassy (posts by actual applicants) so I am going with the newer version with no Notary required.

As always, YMMV.
