Help Needed: What's the Next Step After Mailing Notification of Applicant Readiness

I'm applying for non-immigrant K3 visa in Manila, Philippines. I have sent the notification of applicant readiness together with ds-230 last month. We already have all the required documents. My husband and I are really worried because we need to be in the US on Dec 2012 (target date: dec 15), so we called the embassy last week. They told us to pay the appointment fee. I already set the appointment on Friday (Sept 7) and went to St. Luke's for my medical exam this morning. But the guy was asking for approved k3 visa letter. I decided to go home and cancel my appointment. Worried that the embassy might reject my application without the letter of approval.

I'm really confused. DO we need to wait for the embassy's go-signal before we set an appointment or can we do it ourselves? We're just very worried, because we feel like we don't have enough time.

Once you set an appointment on the website, you receive an appointment letter (via email? via postal mail?) from Manila IV.
