I need to make an Affidavit of Discrepancy for my 7 year old. She has a different last name on the immunization record as in her birth cert.

I don't believe that there is a discrepancy affidavit. Below is the link for the affidavit itself.

I don't recall seeing anything that particular on this web site. I think you'll need to go to an office in the PH to have that corrected. The form I saw posted (DS 3032) is about naming your agent for correspondence with NVC.
