Fiance received a DS 2000 in his Packet 3, does he fill that out.

Hmmmm...never heard of it. We didn't have that one in our check list. And I'm not seeing too much about that form on-line either. But maybe a likkle bump will get more checking this post out. :) Now.... :ot2:
