Here in the USA, we write the given names first, and then we write the family name last. That is is how we address the envelopes when we send letters in the mail. The DS-3032 is basically asking for a mailing address. You should write the family name last when writing someone's address, and also when filling out all US forms that don't specify where to put first (given) name and last (family) name.
