hello all,

i know on the forms that it says to list alternate names(i have two middle names, but think only one is legal) but i didnt put both in beause i thought it would be confliting...my SIN has two middle names, my birth certificate (which i lost ) i think only has one middle name and on other cards and such it has either or...i know i probably should have put them on the forms will this cause major issues later?

Who are you? Petitioner or beneficiary? If you are the beneficiary, you definitely will need your birth certificate. If you have your name in different ways, it may cause additional background checks to clear the name issue up.

Be consistent with your name...

yes i am aware that i need my birth certificate which is why i have already applied to get a replacement..and yes, i am the beneficiary. its not exactly my "fault" i technically have two middles names but some of my legal things only show one or the other.. some show both! i only put the first middle name on my form.

So from now on should I put both of my middle names or just stick with the one?

Independently of the names use normally use in your every day life or other documents, you should always write your name as it appears on your birth certificate and passport (I'm pretty sure they are written the same way). So, from now on, do it as it shows there good.gif
