I have a few questions on the DS-156 application. (My fiance is the beneficiary and I am filling out the forms for him)

question #2 it says place of issuance on the passport it doesn't give a city just the country I am assuming I just write the city my fiance went to drop off the information and pick up his passport.

question #16 my fiance doesn't have a home or fax number do I just write none or leave it blank?

question #18 it asks for spouses name. My fiance is divorced do I write his exwife's information there or do I write none since at this moment he is legally not married?

question #25 it asks for my (US citizen's) work number should I write the number that goes to my office or the main number so in case they call they can reach the secretary and possibly my boss if needed?

question #34 they ask if anyone is traveling with my fiance from Chile to USA. Is this only for other ppl applying for visas to the USA or should I add my name since I will be traveling form Chile to NY to be with him?

question #40 my fiance doesn't speak english so I am asking him for the information and filling everything in. This question asks if someone else prepared the application. Should I write that I prepared the application and then if they ask why I did I mention its because my fiance doesn't speak English?

you can get a DS-156 example form to help you on this page look for giudes examples.

I got the form already but it was for someone who was never married before. These are my questions after reading the directions and looking at the sample form.

#18 asks for spouse's full name (even if divorced or separated)

Hence she should fill in his fiance 's ex spouse name.

Aug 2009 - Met online

18 Mar 2010 - Met up in person for first time in CA. Best vacation ever!

21 Apr 2011 - He proposed!

28 Mar 2012 - Sends in I-129F

02 Apr 2012 - Received NOA1.

05 Oct 2012 - Got RFE.

10 Oct 2012 - Sends in RFE as requested.

29 Oct 2012 - NOA2 after exactly 7 months!!!
