Attachment for DS-157

Hi, i'm doing final preparations for the documents for interview.

I will have an attachment for DS-157 to answer some of the items completely -- it doesn't have much enough space to put in information on some of the boxes; for example, address boxes, and travel arrangement itinerary, etc seem very small for all the information to be put in.

For the DS-157 attachment, does it need to have signature (the main form doesn't require the signature)? Does it need to have the name of the applicant and A# on top of the attachment page? my plan is to just staple the attachment to the main DS 157 form.

did you use a form that has barcode? or was it handwritten? no you don't need to put name on the top of your form nor put your A#, just fill it up.

The DS-157 I have has no barcode in it. Is there another variety of DS-157? The answers that I've put in the form are typewritten using Adobe Acrobat.

Ok I almost forgot, :bonk: :bonk: I was talking about ds-156...anyway if there's no line for signature then you don't have too.

If you need more space for address use a second sheet of paper and reference which box the info applies to.

Travel intinerary? #18 ? Hopefully you are checking NO for that question. They are not asking about your travel to the USA after your visa is approved.
