hi! im trying to register for st. lukes for my fiance's medical. what do i put on POE and date of arrival? I hAve no idea what to put it since im not sure where its goin to be? any note on this?

I don't think it's a big deal.. I put mine for September 2012 (no specific date) and POE is California but I will have my POE at Seattle..

ok. can i do the pre reg for him while in USA or he has to do it himself since he needs to print it out right away? thanks!

don't worry too much about it sis. I just put mine California. And estimated date of departure is September 2012. It's not that important sis. Just put wherever is near to your finance/hubby. and you're fine. :)

It's up to both of you.. :) when I made mine, my fiance was guiding me.. I was talking w him on skype while I am filling up the form.. :)After making the pre reg. SLEC will also send you an email w the same details as what you filled up..

i did mine. and i received email that can be print out just in case he lose the preregistration form. when is he going to st. Lukes? im going there this sept. 04...

Upon arrival at SLEC, applicants WILL BE ACCOMMODATED ON A FIRST-COME, FIRST-SERVED BASIS. NO APPOINTMENT IS NECESSARY. This online registration form aims to reduce registration time by having most of the applicant's information ready upon arrival at the registration area. Pre-registered applicants should only need to have their biometrics captured upon arrival at the reception area.

Click on the link for online registration. Print two copies and show these to the on-duty guard.

yes, you just need to show up there. the online pre-registration will save you time from filling up forms at SLEC. You will still have to fill up something but not that much. just 1 form i think. the ONLINE applicants have their own window/line too at the 1st floor.

I had mine last august 13 which was a Monday. Pre registration online helped me save time. I got there 630am they gave me #40 while those with NO pre reg online the number was around #120++
