AOS I-693 only vaccination sheet w/ x-ray CD

Hello,

My wife and I are about to file the AOS package for my lovely wife :thumbs: but there is one thing I am trying to understand.

I am supposed to send a copy of her I-693 form when she did her medical testing in S. Korea (less than a year ago).

The only form she has is the vaccination page and the x-ray CD.

What about the rest of the I-693 package, do we need to fill that out or can we just send a copy of the vaccination page?

Also, do we need to send a copy of the x-ray CD?

And lastly, if we do need to fill out the I-693 form, most of it needs to be done by the doctor, should we make a new appointment to get it done?

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

I updated my timeline.

Sorry I should have made it clearer. We were approved for a K-1 visa (after getting it expedited due to USCIS mishandling the first one) and my wife received her K-1 visa mid June. After that we married mid July in the USA. And now we are trying to file her AOS.

What does the vaccination sheet say at the bottom? Does it say "complete" or "may be eligible for blanket waiver"?

If so she may not need the I-693 at all.

The CD is not needed - that is for her to keep.

Basically, if her vaccination requirement are all good, she doesn't need to see a civil surgeon here, and just need to send a copy of her DS-3025. If she's missing vaccines, she will have to see a civil surgeon here, get the missing vaccines done, and have Part 1 of the I-693 completed + the vaccination record page by the CS.

Perfect thank all of you guys.

I read the FAQ when my post was moved to the right section and it confirms what you guys have wrote.

The vaccination sheet we have is the DS-3025. I will include a copy of that in the packet.

I could not have figured out how to file all these forms correctly if not for the good people here and those who created this site.

I am so much appreciative.

EDIT: oh no, i thought it says complete but it says: "Vaccine history incomplete- Applicant may be eligible for blanket waiver because vaccination not medically appropriate"

Basically, if her vaccination requirement are all good, she doesn't need to see a civil surgeon here, and just need to send a copy of her DS-3025. If she's missing vaccines, she will have to see a civil surgeon here, get the missing vaccines done, and have Part 1 of the I-693 completed + the vaccination record page by the CS.

Look at that link again: http://www.visajourney.com/forums/topic/335754-ds-3025-complete-or-not/page__p__4971318#entry4971318 and try to see the reason why the doctor picked "incomplete".

Read through what it states in the post that was linked above. If all the required vaccinations are marked off on the DS-3025 and the rest are not age appropriate, then you can just send a copy of the DS-3025 without seeing a civil surgeon for the I-693. They mark it incomplete because all the vaccinations on the sheet were not given, BUT all the vaccinations are not actually required because a lot of them are not age appropriate and that is what the blanket waiver is for. It basically means they are given a waiver for those vaccinations because they are not needed for their age group. The required vaccinations for AOS are MMR, Tdap/TD, varicella(or will be marked VH if you had the chickenpox before) and a flu shot(only required during flu season).
