Just when I think I have things figured out, I have more questions. I just received packet 3.5 from the Embassy in Port-au-Prince. In the list of instructions, it says nothing about a letter of intent or proof of ongoing relationship. And for the affidavit of support (I-134) it just has in parenthesis "recent tax forms" as evidence along with the affidavit. So do I just submit proof of ongoing relationship and letter of intent anyway? And just to confirm, my fiance needs to submit the affidavit with my original signature and employment verification letter with original signature (which means I somehow have to mail them to him in Haiti)?

As for the medical exam...we're going with Dr. Buteau per the recommendation of other VJers. I am right in that my fiance doesn't need an appointment, but can walk-in and get the exam? According to the paper from the embassy, it's $150 + the cost of any additional vaccinations, if necessary. Is that correct?

I would include the new letter of intent regardless, it is to re-assure them that you are still going with the process. It is part of the ongoing relationship. For the I-134, if your income is high, it is enough to just include your w2s, returns and employment letter, if not, include your bank statements and other assets. Yes, original siganture. I think DHL can get them to her in about 3 days.

no appointment is needed for the medical, just go there before 11:00am. yes, Buteau is $150 if you do not need vaccinations. Don't forget $240 for the visa fee to be paid at SogeBank and $20 for the messenger service.

If I were you, I would email or fax her the packet 3.5 and have her do her medical. While she is waiting for the medical result, I would DHL her the other forms. It may take about a week for her to get the medical result.

the new letter of intent is basically the same letter you sent to USCIS. just add a re... example if the original said affirm, new one re-affirm; assure, new one re-assure. That is what I did.

Thanks! Is it okay to just send 2011 tax returns? I don't have W2s because I worked in Haiti for part of 2010 and 2011. I just started a new job in the US a month ago. So on the I-134 form should I put N/A for the questions regarding amount on deposit is savings banks, personal property, stocks, etc. since I make well above the minimum required?
