1. let say the annulment is done,,does my name automatically go back to my single name?or i still need to request for it?also is there any documents needed regarding the annulment or the annulment paper itself is enough?..

2.is it ok to fill out and print the form then put ''date'' as hand written or need to be all computerized?

I would say you need to list any names you have been known by, regardless of whether you are still called that or not. That would mean to include your married name if you were ever legally called by that regardless of the nullification.

Total elapsed time between filing I-129F to GC in hand 569 days or 1 Year, 6 months, 22 days.

 The form is asking for all LEGAL names you have gone by so this would include your former married name. Annulment should be enough to show you are free to marry. If you would like to add to this, it isn't going to hurt to provide more evidence. I can't tell you if your name "automatically" changes with annulment. You can fill in the forms and date them by hand. Just be sure to write legibly. Use black ink.
