My man didnt get anything until a few days before the interview. I didnt even get the email you got until I asked for it.

I'm not sure about any medical form. My guy didnt bring anything (except his appointment letter) to his medical. He didnt have any immunization records. He just showed up at Hope Medical.

You don't need the medical form. When you go for medical they will provide you with one. I hope your fiance have immunization record because they charge alot of money for shots at the medical office.

I received the email with the interview date two weeks after I emailed the DS230 form to the Embassy. And then today he received the Packet 4 (two days after the email). It is my understanding that the medical appointment only requires that he brings his passport, 4 passport photos, the interview appointment letter, any immunization records that he may have and at least $23,000jmd( which is $9,900jmd for the medical exam and the other $13,100 depending on the immunizations needed).
