Hi everyone,

I am wondering if I have to submit the form I-693 with my AOS package or not? I am a K-1 Visa holder and I was told that I do not need any additional vaccinations.

I have a copy of the vaccination record form DS-3025 from the doctor in Denmark when I had my medical examination. I think I have to send that with the package?

Sorry if this question has been asked many times, I did some research and got kind of mixed answers whether the DS-3025 needs to be transcribed to I-693 by a civil surgeon or not.

We provided all the medical information to the CS after our marriage, he did the exam, a TB test,blood test and the forms, we submitted as the CS completed. No issues with-ours, is that what you wanted to know?

I actually checked out the package I sent for my husband. We enclosed the envelope we got from the Civil Surgeon during his medical exam for K-1 as well as a copy of the DS-3025. I labeled them with a coversheet in the package stating such. No RFE. We did not submit another I-693 as it is redundant (the envelope contains the completed I-693). Now I'm wondering if everyone gets a sealed envelope of if his Civil Surgeon just is smart enough to think ahead?

Total elapsed time between filing I-129F to GC in hand 569 days or 1 Year, 6 months, 22 days.

If you had all the required vaccinations at the time of your medical and they are all marked off on your DS-3025, then skip the civil surgeon and just send in a copy of your DS-3025 : http://www.visajourney.com/forums/topic/335754-ds-3025-complete-or-not/page__p__4971318#entry4971318

The panel physicians overseas do not fill out an I-693. That form is used in the US by civil surgeons.

I never got any envelop from the civil surgeon during my medical exam for K-1. They sent the result of the medical examination directly to the US consulate where my interview took place. The radiologist did give me a CD with my chest x-ray and the doctor gave me a copy of DS-3025 though. I don't know if my medical exam results in in the big brown envelop that they had you carry over to the USA, I hope so.

You can just submit a copy of your DS-3025. Your K-1 medical info should have been in the envelop you gave at POE, and if all goes well, it is then transferred to the USCIS.

It can happen that your medical info get lost at POE, but it's rare. In this case, you will have a RFE and will have to go to a civil surgeon.

I submitted only a copy of my DS-3025 in my AOS package, had no RFE. Got my AOS interview yesterday, the IO looked at the paper and didn't say anything about it. :)

You can just submit a copy of your DS-3025. Your K-1 medical info should have been in the envelop you gave at POE, and if all goes well, it is then transferred to the USCIS.
