Kindly explain me shall i mention with from my date of birth year or from the age of 16

The start date is the start date. Answer accurately. If you lived in the same city your whole life, there will be one entry and your month and year of birth will be the start date.

City province country from /to or present

HD AD ind Aug /2010 to present city province and country is same

dfe jef Ksa july/2008 to Aug/2010 country is different

HD AD ind jan /2006 to july/2008 city province and country is same but different street name

HD AD ind july/1996 to jan/2006 ( city , province and country is same but street name is different

Knr AD ind oct/1982 to july1996 ( i mention Here from Birth year cityis different but province and country is same

Answer the question asked. No address is asked for. If you live at ten addresses in the same city, that's one city with a start and end date for that city. This is not complicated.

Think of answering in periods of time. If you leave a city and then come back, you are going to enter that city and time frame more than once. This is like filling out a job application with employment or residence history. If you don't understand the concept, perhaps the petitioner can help you with this.

Do you belong to any professional associations. If you do, list them. If you don't, enter "NONE".

I've already answered this question. Unless you moved to a city on your date of birth, you will be entering the actual month and year you began living in that city, even if it is before you reached age 16. Why is the US Citizen petitioner not helping you with this? These are the simple questions on the form.
